Staff Management – A Key Factor in the Care Coordination Process

In any healthcare environment, it’s safe to say that care coordination and staff management are closely linked and work together in creating a successful, high functioning medical practice.

The key to the success of any medical practice is patient care above all else. When focus is placed on streamlining administrative tasks, more attention can be placed on ensuring only the best care is provided to every patient.

Why is Staff Management Important?

Due to the nature of most medical practices, it’s important that all staff members are aware of their individual roles within the team, in order to keep the practice running smoothly and successfully.

Proper staff management procedures involve the following aspects:

  • Ensuring that all staff are well trained and equipped to carry out their designated functions
  • Knowing how many staff members are required per shift and in which departments – especially in hospital environments
  • Providing supervision and leadership while intervening in areas which need development or further training
  • Implementation of systems such as electronic health records, stock record systems and other staff management software which can help monitor daily functions while detecting, avoiding and warning of potential problems

What is Care Coordination and Why is it Important?

Care coordination is the process of organising all appropriate patient care activities by sharing the relevant information among every person involved with the care process in order to ensure the highest standard of care is delivered.

With effective care coordination, the needs of the patient are communicated to the right people in the right department, ahead of time, ultimately streamlining and enhancing the patient care process.

High-quality, effective and appropriate care are at the forefront of care coordination, making it a key strategy to building a successful medical practice.

Furthermore, the benefits of care coordination lie in the results that come from successful treatment of the patient’s condition.

Challenges Faced in the Healthcare Environment

There are numerous challenges in the healthcare environment that further support the need for care coordination systems and strategies.

These challenges include but are not limited to:

  • A large divide between the public and private sector, with varying treatment facilities and areas of speciality. In other words, a patient who does not have access to a medical aid may require a specific treatment or procedure which is only available in the private sector, making it inaccessible to the patient.
  • Patients are not always supported throughout the process of being referred from a primary healthcare provider (general practitioner or dentist) to a specialist (physical or maxillofacial surgeon). Patients are often unclear about how to make appointments with specialists and how to proceed after they have seen the specialist.
  • Primary healthcare providers are reliable in providing referral information to the specialist when necessary. However, fluid communication from the specialist back to the referring practitioner is rare. This restricts the primary doctor’s awareness of the final diagnosis, prescribed treatment and future procedures, ultimately limiting the level of care the patient will receive after seeing the specialist.

What are the Functions Included in Care Coordination?

The following are activities that can be looked at during care coordination processes:

  • Medication management
  • Teamwork
  • Health information technology or resources
  • Establishing and agreeing on accountability and responsibilities
  • Providing ongoing support for the patient’s self-management
  • Making thorough assessments of the patient’s needs
  • Communication knowledge or information with the relevant staff members
  • Providing follow-up support especially when the needs of the patient change
  • Designing a care plan that is innovative and proactive
  • Assisting with transition of care between primary and specialist healthcare professionals

When looking at these activities, the connection between staff management and care coordination is clear.

Without efficient management of staff, the care coordination process becomes practically impossible.
